Find care
Where can a care search begin?
Choose a way to get an assessment before choosing a product. An assessment is a health review with the person giving care.
Can care happen in a room or by screen?
An in-person clinic and a licensed telehealth service are categories to compare. Care provided from a distance, such as a video visit, is called telehealth.
Under Arizona rules checked on October 3, 2026, the clinician must consider whether remote care can meet the needs of the visit.
Ask whether a hands-on exam or a test is needed, and where it will happen.

Arizona: deciding whether remote care fitsWhat name is useful to ask for?
Ask for the person who will assess you, their licence type, and the name that appears in the board record.
In Arizona, MD doctors, DO doctors, nurses, and physician assistants have separate licence boards. Each board keeps records for its own care profession.
The board websites for looking up those licences were checked on October 3, 2026.
Does an intake form finish the assessment?
An intake form is a set of questions before care. It may help you share your history.
The Federation of State Medical Boards publishes advice for medical boards. It says a form with a set of questions and answers that never changes cannot, by itself, provide a full health assessment.
Its advice, checked on September 26, 2026, allows forms that change their questions based on your answers. The clinician must also be able to seek more health history or ask further questions. State rules decide what is legally required.
Ask how you can speak with the person reviewing your answers.
What if the answer is no medicine?
Ask how the service handles a visit that ends without a prescription. Include the fee and the next step in that question.
A clinician called the prescriber issues the medicine order known as a prescription. It is something to discuss as part of care, rather than the whole reason for a visit.
